If I pay $115 a month for health insurance and go to the doctor 3 times in one month, and a little bit over the next few months, does my premium go up?

No. It's not like car insurance, you don't get a "surcharge" for filing claims. As long as you keep that policy, your rates will only ever go up, when they raise the rates of EVERYONE in your class - or when you have a birthday, and are old enough to jump up to the next class.
