How do you get a health insurance company to pay for a claim after it was initially denied?

My wife was having severe headaches and feared she had a serious medical problem. I took her in to the emergency room, they gave her a shot and a perscription and diagnosed it as sinusitis (sp). The insurance company is claiming the visit was unneccesary. To me, it seemed a necessay precaution. What can I do to get them to pay up?
--------------------
It's common practice to deny claims like this, especially when the ER billing came through with a diagnosis of sinusitus. But you need to appleal this.. in writing. Call the number (should be oon the back of your card) get the address, and lay out the facts... Your wife had been suffering with this for approx 8 - 9 days.. you both felt that it would pass.. on the 10th day she was in sever pain, after attempts to contact her family physician your only other option was the ER, as you had no idea what was wrong with her. After taking her to the ER, they were able to diagnose her. This was a situation that , in hindsight, we should not have tried to allow your wife's imune system to handle on her own. But you had no idea that it would worsen as rapidly as it did. Enclosed are the bills for service. According to the medical plan coverage does exist for these charges. Please review your decision to extend coverage. You may have to 2nd and 3rd request it.. Make sure you cc: the hospital, doctor, etc.. so they see that you have it under control. The company is a good one.. It's just common practice. However, if you were a doctor and could have figured out what was wrong with your wife.. you wouldn't need them. :)

Can you get reimbursed for health insurance payments if you are an independent contractor?

I am an independent contractor (real estate agent) and just wanted to see if it were possible to be reimbursed for my monthly insurance payments. I sell for a developer, but I am not employed by them. Thanks!
--------------------
No. Being provided health insurance is an employee benefit. If you are not an employeee, you don't get the benefits. You can use the cost as an itemized deduction on your income taxes.

What the heck is a health insurance exchange?

And why on Earth is it a major component of all Democratic plans for a government takeover of our health delivery system?
--------------------
A health insurance exchange is an organized marketplace for the purchase of health insurance set up as a governmental or quasi-governmental entity to help insurers comply with consumer protections, compete in cost-efficient ways, and to facilitate the expansion of insurance coverage to more people. Exchanges do not bear risk themselves – they are not insurers. Rather, they would contract with private insurers and possibly offer a public plan option to cover specified populations (such as those obtaining coverage through small employers and those without employer coverage).

I'm 18, getting my GED. and living at home. will I still be on my parents health insurance?

Hey, I'll be turning 18 in about three weeks. I'm planning on getting my GED and I'm a junior in HS. I wouldn't be starting college till at least the fall (September, and its December now) My question is, would i still be on my parents health insurance as I am not in school?? Thanks.
--------------------
It depends on their insurance company. Most however, allow children to be on the poilcy until age 22 and then they start asking for transcripts to make sure that youre in school. As long as there's not too much of a gap between when you get your GED and when college starts, you should be fine.

How soon does dental and health insurance apply in the military?

does it depend on branch? surely they all have universal health/dental care?
--------------------
It will depend on what you are, Active Duty, dependent or Reservist/National Guard. If you are AD then as soon as you leave for Basic/Boot/BCT/Recruit Training and your information is entered into DEERS you are covered medical and dental on base facilities. For AD military there is no charge for medical or dental. If you are a dependent, then as soon as your information is entered into DEERS by your spouse/parent then you are enrolled into Tricare Standard. With Standard there is a yearly deductible and a co-pay for each visit with the doctor/clinic/hospital that is approved by Tricare. After you get your military ID card then you can enroll into Tricare Prime or Prime Remote. With Prime if you register before the 20th of the month then it starts the next month (ie enroll March 19th, starts April 1st). If you register after the 21st then it doesn't start until the following month (ie March 24th, doesn't start until May 1st). With Prime you are assigned a PCM (primary care manager) at the military clinic or hospital closest to you. Now with Dental that is separate, the company you go through is United Concordia and it is $13 a month for single or $23 a month for a family. You are covered for 2 cleanings & 1 xray per year. Then all other work is co-pay up to $1200 for each person per year. You have to pay the first month out of your account and then after that it is deducted from your pay. With National Guard and Reservists along with their family members it is totally separate unless you are Full time then you are covered under the regular Tricare and United Concordia.

How to get health insurance for senior US resident living in Houston, Texas?

She is 68 years old, has lived in the US as a resident for one year. She is not eligible for medicare or medicaid because she is not in the low income target population and for medicare she has to be living in the US for at least five years. In Texas, like many other states, they don't sell private insurance to seniors. Does anyone know of any kind of insurance that we can buy for her, public or private?

Can my husband quit his job and I keep the health insurance?

My husband wants to switch jobs, but I am pregnant and for complex reasons need to stay with my insurance (through his work). Can I stay with the same insurance indefinitely if he quits and not have my plan/benefits change at all?
--------------------
No more than likely not. His employer is giving him the benefits, so if he is no longer with the company then he doesn't get insurance from them. Unless they were super friendly and oddly helpful.

Can someone please help me with a question about affordable health insurance? Please some info?

Where could a 56 year old female find some legitimate affordable health insurance? I would appreicate any advice you could pass along. Thank You so Much!!
--------------------
At the age of 56, "affordable" is going to be relative. IF you are in perfect health, and not overweight, a GOOD low/no deductible plan is going to cost you about $600 to $700 a month. The less healthy you are, the more overweight you are, the more expensive the policy is, and at that age, it won't take a lot for the insurance company to flat out refuse to write you. You really need to sit down with a local, independent agent near you (find one at www.iiaa.org) and let THEM guide you, after asking you a bunch of questions. But if you've got a ton of maintenance prescriptions, and you're looking for a full coverage no deductible plan for $100 a month, it doesn't exist. If someone on the internet offers that to you, IT'S A SCAM.

Are there any health insurance companies that offer policies that are under $100 a month?

I already have insurance with Blue Cross Blue Shield, but it's still a little over $100 a month. I really don't want to have to cut out insurance all together, but I may have to.

How can someone with no health insurance on a low income get mental help?

A friend of mine is in dire need of mental help. Therapy. Something. Anything. Constantly morbidly depressed and miserable. They have no insurance and cant afford to see anyone. Are there any programs for something like this?
--------------------
Yes, every state has free/sliding-scale mental health clinics. They are usually not listed where you can easily find them, so it's best to call the emergency room or the police department to get the location and/or phone number of the one nearest you. No one needs to suffer with depression, anxiety, or any other mental health problem. There is help out there, you just need to make the calls to find it.

Why is my health insurance premium higher than my rent?

is it because health insurance companies are run by the same greedy people who also run universities?
--------------------
Word of advice; shop around and get a better policy. When I was a single parent, I had insurance that covered myself and two children and did was no more or even the same as my rent. Or is it that your premium costs more than you rent simply because you still live under mommy and daddy's roof?

Is the soution of health care reform by elimite the healthcare insurance provider?

Would it be better answer just elimite the health care company (the middle man) that way people can paid only when is sick, and government regulate the cost. This also benefit doctor since they don't deal with the annoying insurace company and be more productive and reduce cost. Many country has no health care insurance but still provide very affortable surgery etc..
--------------------
Eliminating the insurers is not the issue. The high cost of healthcare is actually not an insurance carrier issue like most people and the politicians are making it out to be. The fact is 99% of premiums collected are used to pay claims. The carriers make their money off the stock market like the banks do. The problem with insurance is more of an issue of a lack of quality doctors, since there are not enough doctors, the prices for services are inflated. The other major problem is rampant fraud that occurs. I know of one woman who was frauding medicare for thousands of dollars a year and getting them to pay for the transportation for all of her day trips to the mall until she got caught and that is just one of the many.

Are we going to start arresting the homeless for not having health insurance?

With Obamacare, everyone now has to have insurance or pay a fine. If you are homeless, with no income, you can't buy insurance or pay the fine. Will we be putting these people in jail with the dangerous marijuana smokers? What about if someone is Amish, and their religion forbids them to buy insurance?
--------------------
No. Just the middle class workers that will be hit the hardest. Pretty much like normal.

How can I get health insurance if I am immigrating to the US?

I am a US citizen moving back to the US with my husband and 2 children. My husband is the one who is immigrating - the kids and I are citizens. However, we've been told that we can't get insurance for 1 year. Does anyone know how to get insurance in this situation? Healthcare is so expensive in the US - what if we're in a car accident or something? I'd be afraid to take my family there if we weren't covered for insurance. A fried of mine is even an Allstate agent and she said that she couldn't find a carrier that would cover us. My husband will have a green card, social security number and permanent residence. Any advice from anyone on this subject would be greatly appreciated! Thank you.
--------------------
I think the waiting period is untrue. The reason I say this, is that I had insurance right after I applied for my green card. I was working on an EAD and Pacific Source gave me medical and dental without any hassles.

How can a married couple who cant afford health insurance get assistance?

We are in our late 50s, both work, but cant afford to pay for health coverage & I am a diabetic
--------------------
Well, if you're looking for someone else to give you assistance, you'll probably have to wait until you qualify for medicaid, or turn 65 and can get on medicare. Many churches have an assistance program, but it's really aimed at one time assistance, not ongoing stuff. My best recommendation, is that both of you should start looking for new jobs, that will give you access to health insurance benefits. Even an extra part time job at Walmart would get that for you.

Health insurance in case of an accident?

If there were an accident, would my Health insurance pay for it or does my car insurance. How is this decided? My car insurer asked me to indicate the primary healthcare provider in case of an accident (them or the health insurance company). My Health insurer on the other hand says that it would be shared between them based on the type of accident. Would it change anything if i indicated that the car insurer will be primary provider? would it deny me the benefits of health insurance?
--------------------
It sounds like you are in NJ where it is optional on your auto insurance to make your health insurance primary. I do not know what the savings is now, but when I was there it was minimal & not worth it (I think it was somewhere around $25 or less per year). Get a quote to see. Remember, health insurance usually has a lifetime maximum ($1,000,000 or $2,000,000). If you have a major illness & a major accident, there goes your health insurance. PIP coverage in NJ is per accident limit (used to be $250,000 max per accident) so technically, you can have many accidents over the course of your policy & the insurance company gives you the $250,000 each time. Also, Medicare never pays primary, that is something to think about if you have an elderly relative in your vehicle who does not have their own auto coverage & no auto coverage in their household because their medical would then come from your policy. You can always file your deductible & copayments from your auto PIP coverage with your health insurance, that is what your health insurance was probably talking about. If you are looking to save money on your auto insurance, take higher comprehensive & collision deductibles. This will give you the most savings in the long run.

Fiance visa to america and health insurance?

My fiance is travelling to California on a fiance visa this month. We have 3 months to get married and then we can apply for his permanent residence and social security, which I hear will take about 6 months. Therefore I don't think I can put him under my work's health insurance. In the meantime, what can he do about health insurance? Will travel insurance w/ medical coverage be okay until I can put him under my health coverage?
