If you exclude yourself from health insurance, does that mean you exclude yourself from health care?

I think that's an important distinction. Without an insurance mandate there are many people who chose not to carry insurance. But that doesn't mean they chose not to receive health care. True - some people without insurance could pay for their own health care. But most could not which leaves the tax payer to cover their bill.

No, it just means they get gouged when they decide to see a doctor. "...hospitals all over the country charge their highest prices, by far, to those who can afford it least — the 46 million Americans who don't have health insurance... Hospitals charge uninsured patients two, three, four or more times what an insurance company would pay for the same treatment...."
