Getting daughter on health insurance?

The state is court ordering my bf to put our daughter on his health insurance or a different one, she is currently on medicaid. He's asking me to put her on it so they don't have to go to court but I'm not quite sure how to go about doing that i've never had to switch an insurance provider before. The insurance issomething called Coxhealth...individual ppo somethin rather idk. I'm a bit confused on how to get her off medicaid and onto this other health insurance. Any suggestions?

First, the insurance company is Cox Healthplans. CoxHealth is something else. Second, if the daughter is a newborn or an adoptee and it is an individual plan (not a group plan), go to http://www.coxhealthplans.com/pages/js/p… print the form, complete section I, skip section 2, check box B in section III, complete sections IV and V. Send them the form. They will do section VI. Third, if it is a group plan from work (either your job or his), go to http://www.coxhealthplans.com/pages/js/p… print the form, complete Section A, check box "Add Dependents" in Section B, in Section C check whichever box on line one applies and either "employee & child(ren)" (does not cover employee's spouse) or "employee & family" (covers children and spouse) on line 2, skip section D, complete section E through the end of the form, and ask HR whether you give it to them or mail it to Cox yourself. If it is an individual plan (not a group plan) and the daughter is not a newborn or an adoptee, I do not know. Call Cox and ask them.
