The quality (and amount of people who actually care) of the United States healthcare system is comparable to that of third world countries. why? how has it gotten this bad?

It is a for profit system. Corporations have repeatedly proved that they are not good stewards of the public interest. Did you notice that the AMA got tired of doctors being ranked by insurance companies so they turned the tables and started ranking health insurance companies. More interesting is that former Senate majority leader Bill Frist's company, United Health was ranked dead last by the AMA.
