Where to find a copy of a health insurance policy?

I would like a sample of a health insurance policy.

You will have to ask your agent for a sample copy. If you don't have an agent, and don't have a policy, you'll have to ask someone who does have it, for a sample copy. Or walk in to an independent agent's office, see if he has a sample policy he'd be willing to hand over to you. Policy forms are not published on the internet.
