Is Clomid covered by most health insurance providers?

Does anyone know the estimated cost for someone with health insurance?

I found one article (link below) that cited a survey showing that 44 percent of private insurers and Blue Cross Blue Shield plans cover clomid and 67 percent of HMOs cover clomid. However, I could not locate the original survey and it is from 1993, so the data is probably obsolete. I would think that it's gotten easier to receive infertility treatment under your insurance plan since 1993, though. The International Council on Infertility Information Dissemination has a page (link below) on possible insurers that will cover infertility treatments (through employers). I've also included a link to a page from the American Fertility Association that lists recommendations about what to look for in an insurance plan that covers infertility treatment.
