How many people in the U.S.A. have access to affordable health insurance through their state but have never?

looked for it because they don't want it? This is a serious question because just about anybody can get state government care in my state but most don't ever apply until they're already in the E.R. You can get rejected if you make too much but once again if you make that much you could have already purchased health insurance if you wanted it.
