i live in PA, and ive heard of thr CHIP program, but whats the best way to apply to get approval, by email, phone or mail? i dont wanna get turned down cuz i missed something. thanx!

I would definitely call. You can get all the information and ask questions as they arise. If you fax or e-mail you'll have to keep e-mailing or faxing if you have more questions.
