If I'm self-employed, where on my taxes do I deduct the cost of my health insurance?

Do I add it into my deductions on Schedule C adjusting my taxable income, or does it go on Line 29 of my 1040. Also: is it just the total costs of my health insurance that I pay for every month, or does it also include medical expenses (prescrip., out of pocket dental not covered, dr visits) ?

Half goes on the front of Form 1040 in Line 29, the rest is a schedule A itemized deduction.
