What are the laws for an employer to provide health insurance in the State of California?

For example how many hours must one work? Is the employer required to provide some sort of plan, is not a contract employee or not an independent contractor?

In CA, employers are not required by any law, State or Federal to provide health insurance for their employees. They can require that the employees be full-time (usually 35-40 hrs per week). Or whatever eligibility they want. BUT if they set up eligibility requirements, they must offer health insurance to all employees that meet those requirements. And, in CA, the employer is required to pay at least 50% of the monthly premium or $100-whichever is less.
