What was the first Health Insurance company?

Can anyone tell me who established the first Health Insurance company in the USA and in what year it was established? Thank you.

Blue Cross is the oldest insurance company. It was founded in 1929 for teachers in Texas. Blue Shield was founded a few years later. The first HMO was Kaiser Permanente, and it was founded in 1945.
